Are you a U.S. citizen, and do you have a valid social security number?

Are you currently working toward your degree or seeking financial aid to do so?

Do you have a high school diploma / GED? or Have you passed an approved ATB test?
  Yes   No

If you’re a guy between the age of 18 and 25, are you registered with the Selective Service?
  Yes   No
